201 Panel: Engaging Patients Effectively in APM Development

This session will feature patient and consumer representatives addressing questions such as what does meaningful patient engagement mean and what does it look like; and what are the essential tools (e.g., training modules for implementation of patient and family councils, shared care planning, shared decision-making, telehealth applications, and others) that consumers and patients need to be informed decision-makers in an emerging value-based payment environment. Panelists will also speak to the existing infrastructure for patient and family engagement, identifying when it is more efficient to leverage architecture that has already been built, and when stakeholders should look to create new pathways. The panel will also examine how health plans are focusing their alternative payment design and implementation efforts on how to best support patients and consumers, thereby aligning payment with principles of patient-centered care. Panelists and attendees will explore the idea that misalignment between payment and patient priorities creates significant barriers to improving care and reducing costs.
