108 Workshop: What Does Success Look Like in Practice Transformation?

Workshops are designed to address specific challenges the field is experiencing in implementing APMs. The focus will be on strategies to overcome these barriers as well as highlighting success factors that could be applied or modified in different contexts or markets. This will be a hands on working session guided by a facilitator along with experts in the field who have achieved success and who will offer their perspectives, as well as concrete steps participants can take to implement what they have learned at their respective organizations.

This workshop will help guide practices – particularly those currently in a 100 percent fee-for-service reimbursement model as well as in early phases of APM implementation – in their thinking about one or more solutions to reduce administrative burden, improve day-to-day practice operations, and better position practices to succeed in APMs. This session will walk attendees through practical solutions to support practice transformation, such as data collection and sharing systems, care model design structures, and other supports. Potential framing for this session can include the following:

  • Best practices for processes such as: using registries and other tools to improve population health; reviewing data received from other practices to promote improvement.
  • How practices can invest wisely in infrastructure, so they don’t purchase more than they need and instead invest and implement incrementally as the transformation process matures.
