102 Panel: Clinician Experiences in Operationalizing Specialty Care APMs

This session will focus on the challenges and opportunities faced by specialty care clinicians who are participating in APMs. Panelists will share their experiences in moving toward alternative payment, including promising practices related to patient attribution, care coordination for patients with multiple chronic conditions, data sharing, and other pertinent issues. The objective of this session is to raise the question of what future iterations of specialty care APMs can and should look like, focusing on concepts and practices that can be applied across clinical conditions and procedures.
