Efforts are underway in many states to implement alternative payment models (APMs) through partnerships with health care provider networks, state departments of human services, state Medicare and Medicaid organizations, and other stakeholders. Though each approach is unique, they all share some common challenges such as finding the best contracting vehicles, working with multiple providers, managing and disseminating data, and sharing their results and success stories with the community at large. In all instances, an effective infrastructure is a key to success. In this session, representatives from four state programs will share their lessons learned and provide takeaways that attendees can apply in their own state programs.
