In this session, the presenters will identify what factors motivate a practice’s leadership team to be willing to perform the work necessary to transform into an alternate payment model (APM). Regardless of the different types of organizations (i.e., Federally Qualified Health Center, large practice, small or solo practice, or large system outpatient clinics), when the leadership team, including the Chief Financial Officer, Chief Medical Officer, and Chief Executive Officer, can agree on patient-centered strategies for every decision, innovation for transformation occurs.
The presenters will discuss how practices need a roadmap showing the new capacities they need to build and help aligning various initiatives that provide assistance to them on their journey.
This will be an interactive discussion with leaders about integrating QIN-QIO efforts into larger initiatives, while overcoming the dual challenges of training/developing effective practice coaching networks and recruiting practices for participation.
