This session will focus on the relatively new frontier of savings opportunities through APMs in post-acute care. Panelists will share their experiences with implementing alternative payment arrangements to improve the care trajectory for patients in PAC facilities; how APMs are being used to successfully – and appropriately – reduce the rate of patients that go from a PAC to a short-term acute care hospital; and how they are working with device manufacturers to use monitoring programs for patients, sharing accountability for both cost and outcomes across the PAC facility and the manufacturer. Discussion will also focus on how collaborative partnerships are formed between PAC facilities and either ACOs or episode payment “quarterbacks,” in an effort to support a positive patient experience with post-acute care.
