Christy Mokrohisky (St. Joseph Heritage Healthcare) Christy serves as the Vice President of Population Health Management at St. Joseph Heritage Healthcare where she oversees design, program implementation and outcome measurement for population health. She manages value-based contracts with varying degrees of financial risk to improve quality outcomes, engage patients and reduce the total cost of care. She oversees quality and performance programs such as MACRA, risk adjustment and commercial pay for performance programs. Prior to her role at St. Joseph, Christy worked at Dean Clinic in Madison, WI. Christy holds a BA and an MBA from Marquette University in Milwaukee, WI.
