Objectives:
In the shift from volume to value-based payment models, there is a significant drive to improve the health of populations through alternative payment models (APMs). To be successful, and to avoid the potential pitfalls of delivering “one-size-fits-all” care, APMs need to incorporate meaningful person and family engagement, reflected in the following actions, with the goal of creating a positive impact on patient care and health:
- Collaborative engagement of consumers, patients, families, and their advocates in the design, implementation, and evaluation of payment and care models
- Proactive communication and partnered decision-making that incorporates the needs, values, and preferences of patients
- Engagement of patients as partners in their own care
- Effective coordination of primary care services with other aspects of care
- Use of technology to support patient and family engagement and activation
