L1: Meet the Leaders

Speak with a LAN Guiding Committee Member, a LAN Work Group Member, or CMS Official

During the interactive Meet the Leaders session on October 25, from 11:15 am – 12:15 pm, LAN Summit attendees will have the unique opportunity to privately speak with a health care leader to ask specific questions about alternative payment model adoption and delivery system reform efforts.

 

Please use the following form to sign up for your 15-minute, 1-on-1 meeting with one of our health care Leaders. When signing up, please keep in mind:

• All sign-ups are limited to 1 person per 15-minute meeting.
• Meet the Leaders appointments are first come, first served.
• Appointments to speak with Sam Nussbaum in our Where Do I Fit in the APM Framework? category will be considered pending until confirmed. You will receive an email confirming your appointment if your submission is selected.
