4D: Advancing the Delivery and Payment of Primary Care

Earlier this year, CMS announced Comprehensive Primary Care Plus (CPC+), an advanced primary care medical home model and the largest investment in primary care practice transformation in our nation’s history. CPC+ brings together Medicare and over 50 payer partners to support primary care clinicians in their effort to deliver better care and improve the health of their patients. This session, featuring members of the CPC+ team, will highlight key features of the model design, strategies for payer engagement, and implications of the Quality Payment Program on primary care reform.
