4C: LAN Committed Partners in Action: Lessons from Leaders in Implementation

LAN Committed Partners are organizations that have shared their own goals in support of APM adoption with the LAN and are publicly acknowledged for their leadership. During this session, a variety of Committed Partners share their experience implementing Category 3 and 4 APMs. Attendees will glean actionable insight from several perspectives: healthcare provider, health plan, manufacturer and private employer. Panelists will describe their implementation efforts, challenges and obstacles, results and lessons learned, leaving the audience with a variety of successful strategies to be applied in their own organization.
