As the U.S. health care system is undergoing a significant transformation, health systems are no longer able to rely on status quo patient care and fee-for-service reimbursement. Traditional reimbursement structures are now shifting to value-based payment models while at the same time, recognition is increasing that community surroundings (i.e., social determinants of health) have a larger impact on overall wellbeing and mortality of individuals and the broader population. During this session, panelists, including leadership from AcademyHealth, will share their findings from different perspectives – providers, plans, and communities at large – about the financial role and capacity of the health care system in addressing these social determinants of health.
