Community-based organizations (CBOs) are vital in advancing health equity within alternative payment models (APMs). Patients with health-related social needs often have strong, established connections with local CBOs, and APMs can foster meaningful relationships between health systems and CBOs.
Moderated by Professor of Healthcare Ethics at the University of Chicago and Health Equity Advisory Team (HEAT) Co-Chair Dr. Marshall Chin, and Market President of AmeriHealth Caritas District of Columbia and HEAT Co-Chair Karen Dale, the panel will provide insights about how payers, health systems, and CBOs have successfully navigated societal barriers to establish impactful partnerships that benefit patient populations. Panelists will reveal the essential standards that underpin successful partnerships that can empower communities and bridge the gap between health care and social services.
